I-SCAN GROUP OF COMPANIES – PRIVACY NOTICE 

I-SCAN GROUP OF COMPANIES (I-SCAN DIAGNOSTIC CENTER, INC and I-SCAN HEALTHCARE INC.) is committed to protect and respect your personal data privacy. Information collected and held by I-SCAN is subject to the Data Privacy Act of 2012, and I-SCAN respects and supports privacy protection in relation to information collected. I-SCAN is committed to comply with the Data Privacy Act (Republic Act No. 10173).  

This PRIVACY NOTICE sets out how I-SCAN GROUP OF COMPANIES (I-SCAN DIAGNOSTIC CENTER and I-SCAN HEALTHCARE INC.) uses personal information of patients and explains:  

  • What kind of information is collected 
  • How it is collected 
  • How it is managed and stored 
  • How it is used 
  • How patients can access and update information relating to them  

This policy applies to information acquired from patients of I-SCAN that have completed the Patient Registration process, other accompanying forms that are required to administer their requests, and/or through I-SCAN’s Customer Service Desk.     

PERSONAL INFORMATION COLLECTED   

I-SCAN collects and holds information relating to patients’ requests. This personal information may include, but is not limited to the following data:

Basic personal information, such as but not limited to patient’s name, address, date of birth, age, sex, contact information 

  • E-mail address 
  • Physician’s Name 
  • Medical History 
  • Relevant contact information of the patient’s relatives, guardian, or next of kin 
  • Vital signs (Blood pressure, temperature, pulse rate, respiratory rate)  
  • Financial details such as last 4 digits of Bank Account, reference code for GCASH, QR PH payments, and other online payments 
  • Other information as necessary 

 Patients are responsible for ensure that all data, whether personal information, sensitive personal information or privileged information submitted to I-SCAN are accurate, complete and up-to-date.   

The I-SCAN is also equipped with CCTV cameras to help us ensure safety and security of patients, employees, and the establishment.    

COLLECTION METHOD  

Personal data is collected through various methods, including when a patient registers at any of our branches and requests services by filling out patient information and consent forms. It can also be obtained through correspondence via telephone, email, I-SCAN’s website at https://www.iscandiagnostics.com/, and messaging applications such as Viber and FB Messenger.   

In addition, when a patient undergoes a procedure, details of that procedure (including medical diagnostic testing results information related to the patient) are recorded and kept in the information systems and/or database.  

Personal data may also be provided voluntarily by patients directly or through a third party who has been properly authorized by the patient to disclose their data to I-SCAN (such as their authorized representative, companies, or third-party healthcare providers). This occurs after the patients (or their authorized representative) have been informed of the purposes for which the data is collected and have given written consent for the collection and use of their personal and medical data for those purposes, or when the collection and use of personal data without consent is permitted or required by law.   

PURPOSES OF COLLECTED PERSONAL INFORMATION 

I-SCAN primarily uses the information in the database to manage and administer the medical diagnostic tests requested by patients, either directly or through their employers or authorized representatives, to fulfill their requests. Additionally, I-SCAN uses the data for secondary purposes, such as conducting statistical analyses, planning and executing marketing campaigns, and developing new products.  

No persons other than I-SCAN, its employees, service providers, and partners are authorized to access patient information held in its information systems and/or database. All persons who can access the information are required to strictly adhere to the principles of data privacy and confidentiality.   

I-SCAN may collect and use the personal data and medical information for any or all of the following purposes:   

  • Verifying patient’s identity 
  • Administering the medical diagnostic tests requested by the patient  
  • Fulfilling obligations related to the services requested by the patient 
  • Transmitting to any unaffiliated third parties including I-SCAN’s third-party service providers, partners and relevant governmental or regulatory authorities 
  • Verifying and processing payments                                     
  • Responding to and processing requests, complaints, and feedback from patients 
  • Managing the patient’s relationship with I-SCAN 
  • Sending marketing information about I-SCAN’s goods or services, including notifications about marketing events, initiatives, and promotions 
  • Conducting statistical analyses, planning, executing marketing campaigns, and developing products

In compliance with applicable laws, regulations, or to assist in law enforcement and investigations conducted by governmental or regulatory authorities, I-SCAN may disclose personal data:  

  • When required to fulfill obligations related to the provision of the services requested by the patient 
  • To government agencies that lawfully collect information, such as the Department of Health, which may require submission of relevant personal data for disease surveillance and monitoring of certain diseases, injuries, etc. 
  • To meet legal requirements, such as when ordered by a court or a government agency or public authority 
  • To third-party diagnostic testing facilities, service providers, agents, and other organizations engaged by I-SCAN to perform any of the functions mentioned above

The purposes outlined above may still apply even if the patient’s relationship with I-SCAN (such as through a contract) is terminated or changed. This continuation can extend for a reasonable period afterward, including time needed for I-SCAN to enforce its rights under any contract with the patient.   

STORAGE AND TRANSMISSON OF PERSONAL INFORMATION   

Patient information registration and consent forms are securely stored at the branches, while digital records are kept electronically in encrypted information systems and/or computer databases managed by I-SCAN.  

SECURITY MEASURES  

I-SCAN is dedicated to ensuring the accuracy and security of information obtained through manual forms and digital records stored in its databases and information systems by implementing appropriate technical, organizational, and physical security measures.

WITHDRAWING CONSENT   

Patients may withdraw consent and request I-SCAN to stop using and/or disclosing his/her personal data for any or all of the purposes listed above by submitting their request in writing or via email to I-SCAN’s Data Protection Officer at the contact details provided below.  

Upon receipt of the inquiry or request, the I-SCAN Group’s Data Protection Officer shall confirm that the inquiry or complaint shall be processed within a particular time frame.  

Although I-SCAN honors the patient’s choice to decline signing this agreement, please be aware that I-SCAN may not be able to continue offering their services, as the information gathered is essential to the services and products they provide.   

Please be aware that withdrawing consent does not impact I-SCAN’s right to collect, use, and disclose personal data where such actions are allowed or required by applicable laws, even without consent.  

Every patient has the right not to receive direct marketing communications that are not included with any statement. If the patient wishes to exercise this right, please send an email request to customercare@iscandiagnostics.com.   

ACCESS AND UPDATING CUSTOMER INFORMATION  

If the patient wishes to review or update his/her patient information held by I-SCAN, he/she may approach any of I-SCAN’s Customer Service personnel at the Customer Service counter at any of the branches or he/she may send an e-mail request to customercare@iscandiagnostics.com.
